Extended hours service

The Brook Medical Centre are now part of Primary Care Plus which is a Community Interest Company (CIC).

Primary Care Plus offer pre-booked extended access appointments for all patients registered at the below practices.

Appointment times

The extended access appointments are provided on weekdays and weekends.

Weekday evenings

Appointments are available Monday to Friday between the hours of 6pm to 8pm.

These opening hours are available at the following “hub” practice:

  • Park Farm Medical Centre, 3 Park Farm Centre, DE22 2QN

Weekends

Appointments are available Saturday and Sunday between the hours of 8am to midday.

These opening hours are available at the following “hub” practice:

  • Park Farm Medical Centre, 3 Park Farm Centre, DE22 2QN

Please note: practices involved have a limited number of appointments available.

How to book an appointment

Appointments must be pre-booked via your practice, please state if you would like an extended access appointment.

When you are booked in for an appointment you will be asked the nature of the problem so we can book you in with the right person. We also request your consent to share your full record.

Without both of these you will not be able to access these appointments.

We are not contracted to see any walk-in patients during these extended access sessions.

What to expect in your appointment

Consultations will be 15 minutes long and will be carried out by a full range of quality, experienced staff. In the future, hubs will be offering a variety of services, this may include:

  • Dressing clinics
  • GP appointments
  • Nurse practitioner appointments
  • Blood clinics
  • Smear clinics
  • Long term conditions clinics

Surgery closures

The surgery is closed for all English bank holidays,

  • Friday 18th April
  • Monday 21st April
  • Monday 5th May
  • Monday 26th May
  • Monday 25th August
  • Thursday 25th December
  • Friday 26th December

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
