Appointments

An appointment is for one person only. Where another family member needs to be seen or discussed, another appointment should be made. You have the right to see the clinician of your choice, but this may not be possible if you need to be seen urgently.

Urgent appointments

To request an urgent appointment for today opening times:

Routine appointments

We offer appointments during the following hours:

  • Monday to Thursday, 9am to 6pm
  • Fridays, 7:30am to 6pm
  • Weekends, closed

To request a routine appointment in advance during opening times:

Phone advice

The nurses and doctors may be consulted for advice over the phone if you do not feel an appointment is necessary.

Extended hours service

The Brook Medical Centre are now part of Primary Care Plus which is a Community Interest Company (CIC).

Primary Care Plus offer pre-booked extended access appointments for all patients registered at the below practices.

Appointment times

The extended access appointments are provided on weekdays and weekends.

Weekday evenings

Appointments are available Monday to Friday between the hours of 6pm to 8pm.

These opening hours are available at the following “hub” practice:

  • Park Farm Medical Centre, 3 Park Farm Centre, DE22 2QN

Weekends

Appointments are available Saturday and Sunday between the hours of 8am to midday.

These opening hours are available at the following “hub” practice:

  • Park Farm Medical Centre, 3 Park Farm Centre, DE22 2QN

Please note: practices involved have a limited number of appointments available.

How to book an appointment

Appointments must be pre-booked via your practice, please state if you would like an extended access appointment.

When you are booked in for an appointment you will be asked the nature of the problem so we can book you in with the right person. We also request your consent to share your full record.

Without both of these you will not be able to access these appointments.

We are not contracted to see any walk-in patients during these extended access sessions.

What to expect in your appointment

Consultations will be 15 minutes long and will be carried out by a full range of quality, experienced staff. In the future, hubs will be offering a variety of services, this may include:

  • Dressing clinics
  • GP appointments
  • Nurse practitioner appointments
  • Blood clinics
  • Smear clinics
  • Long term conditions clinics

Cancelling or changing an appointment

If you are unable to keep your appointment, it is important that you inform the surgery as soon as possible. Appointments are always in heavy demand and yours could be offered to another patient.

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

To request a visit, please contact the surgery as early as possible. Our receptionist will take your details and your phone number as the doctor may wish to speak to you.

Please remember that home visits are strictly for people who are housebound or too ill to attend the surgery.
